This Behavioral Based Safety (BBS) observation checklist helps teams proactively identify and correct at-risk behaviors on site. Sections cover PPE compliance, documentation and RAMS sign-off, communication and leadership, housekeeping, chemical handling, safe movement and access/egress, body positioning and manual handling, and tools and equipment. Capture corrective actions, assign owners, provide feedback, and sign off to drive continuous improvement and reduce incidents.
