Use this behavior-based safety checklist to structure safety observations on any job site. Follow guided steps to introduce the process, ask open-ended hazard questions, and observe work. Verify critical safe behaviors including PPE use (head, eye and face, hearing, hands, body, visibility, and foot protection), proper body positioning, lifting and pinch-point awareness, and correct tool selection and condition. Assess eyes-on-path and hands awareness, travel speed and route, and safe ascending/descending on trucks and trailers. Capture notes, photos, and corrective actions, gain worker commitment, and record additional observations and signatures.
