RMS H4 Confined Spaces Surveillance Checklist

This RMS H4 Confined Spaces Surveillance Checklist supports safe work in confined spaces on civil infrastructure projects such as bridge construction. It guides supervisors and safety officers to confirm risk assessments and SWMS address access and egress, engulfment, and contaminated atmospheres before, during, and in emergencies. The checklist covers identification and control of hazardous activities, air monitoring and equipment calibration, nationally recognised training, signage and physical barriers, rescue and first aid arrangements, entry permits, and continuous communication with a standby person. It prompts assessment of structural soundness, illumination, required cleaning, and comprehensive site inductions including emergency procedures, lockout, safety equipment use, rescue drills, fire protection, communications, and site hazards. It also includes compliance and follow‑up actions (NCRs, CARs, and close‑out).
